Well, you should take care to word the questions so that each question is short and concise and relevant to the person being appraised. If the questions are not relevant it will defeat the purpose of giving the person the motivation to change.

I agree with that. You should match the level of comprehension that the person and the respondents have. As much as possible, use plain English and stay away from jargons.

We also make it a point not to use qualifiers like when necessary or if appropriate. Each question has to be complete and direct to the point.

And you should not forget to also include space for the comments. These comments often provide really valuable insight that a closed set response cannot give.

Right. On top of that, you should also include free text questions. Some sort of essay that the respondent gives on the appraised person.

You should always use a performance scale like Needs improvement, poor, good, excellent instead of scales that uses frequencly like never, sometimes, and always. This is because you are rating the person's skills and competencies, not the number of times that he was given the opportunity to do the tasks.

I think this is the ideal. You should have ample spaces for them to give better feedback.
